For your convenience all the forms that will be needed for your first visit may be completed on-line and submitted to our office or you may download the forms, complete them at home and bring them with you to the consultation appointment.

All forms completed on-line will be sent to a server located in our office which is secure and regulated by all HIPAA requirements.

Please keep in mind if you complete the forms on-line you will still be required to sign the forms when you come in for your first appointment.

Note that due to new regulations ("red flag") we are now required to obtain a copy of the patient's photo id for our records. We request you bring appropriate photo id to your first appointment.
